Functional Capacity Assessments (FCA)

HayleyOT provides detailed Functional Capacity Assessments for NDIS participants. These assessments help document your current abilities and support needs across all life domains, like mobility, self-care, social interaction, and communication.
The resulting report can be used for:

  • Accessing or reviewing NDIS funding

  • Support planning

  • AT and home modification justification

  • Informing support coordinators or plan managers

Assistive Technology (AT) Prescription

HayleyOT assess, recommend, and prescribe assistive technology, from mobility aids and seating to shower chairs, communication tools, and custom modifications. She also helps clients trial equipment, complete NDIS AT reports, and access the right tools to improve daily living.

Post-Hospital Discharge Support

HayleyOT assists clients during the critical transition from hospital to home. Our post-discharge OT services focus on recovery, mobility, equipment setup, home safety, and regaining independence. She coordinates with discharge planners and other care teams to ensure seamless, supported recovery.

Home Modification Assessments

HayleyOT can conduct in-depth assessments to identify safety risks and recommend home modifications such as grab rails, ramps, bathroom upgrades, or layout changes. She helps  create a safe, accessible environment so you can live confidently and independently at home.
